Midway Homes (Long term care)

Problem identified – Increase number of population with various disability and elderly population

Solution – Develop caring and rehabilitation centers (MIDWAY HOMES)

Aim: The key aims of these centers are reduce complications through long term rehabilitation before socialization in need after acute treatment in secondary hospitals.

Method: In first step we identified divisional hospitals with adequate man power and infrastructure for each base hospital to admit patients.

Expected Outcomes:

  1. Early discharge from acute care setting.
  2. Minimize complications and readmission following long term rehabilitation.
  3. Increase availability of rehabilitation facilities in close proximity patients community.
  4. Improve quality of life of persons with disabilities and elderly in the community.
